I'm thinking an apprentice was told to use a "doughnut" to allow a certain sized conduit to fit a larger hole, he decided to be a smarta$$ and that's the result.
Kind of like in Naval Air the Airman Recruits were told to fetch "a bucket of propwash"......or like summer camp "snipe hunting".
