IP camera over the internet?

I've got an IP camera that I use Tinycam Pro with, I plan to use it as a wildlife viewing camera. Everything works great on my home network, but it is not viewable once I leave my home.
I'd like to be able to view the camera with my phone or tablet no matter where I am.
How do I go about setting it up for that?
I've been reading about port forwarding, but I can't seem to make that work.
Please phrase any help like you are speaking to someone who know little to nothing about this equipment. :)

Do not use P2P, uPnP or port forwarding. The internet just loves those "tools" because it makes it so easy to hack the camera and your network. Not necessarily to steak data but to use your equipment in DoS attacks. Of course, if they can steal data they'll do that at the same time.
