I just read this and it completely shredded my soul. This is all about accountability.

Massachusetts VA hospital broke federal law in failed hunt for missing veteran found decomposing in stairwell

Bedford VA hospital in Massachusetts violated federal law by failing to properly search for a missing 62-year-old Army veteran whose decomposing body was found a month after his disappearance in a stairwell just 60 feet from his room.

Wow, this is horrible. Lots of blame to go around. But in my mind, the company that is making a lot of money here, Caritas Communities Inc, is most to blame. If they are providing the facility, and it sounds like they are providing security as in the story it talks about "people behind the desk". How can they NOT check the stairwells daily? Every building I have worked in had someone, security or janitor, check each stairwell every day.

I completely agree with you Samplenhold. My reference was about holding them accountable for not doing a daily head count, hourly, what ever their SOP is. I am sure it is AT LEAST once a day. It is very disturbing and just gross negligence.
