No, there should be a screw on the base with a directional arrow, loosen the screw and rotate the stem in the direction that will then allow the cam to be point to either side....then tighten the screw.
There's also a bracket variant that has that same screw that you loosen for rotating but instead of being on the base as I described above it is on the stem, near the mounting flange; loosen, rotate to desired position then tighten.
In either version, to get the cam to pan sideways (horizontal), the stem will have to be rotated to allow the ball swivel to enter the slotted part and move to that direction; that will also affect the vertical movement, so it's a compromise. It's hard to explain, just put your hands on it and it'll make sense to you.
EDIT: Found these images, the screw that allows the base to be rotated is either on the stem OR down on the base:
