This is the non-Poe port version.

For those who have and are using this model can you tell me how noisy this nvr is? What about the heat output?

In pictures I don't see a fan on the back of the unit... is there a different fan in this model or none at all thus making this near silent?

The fan as you are looking at the front of the unit is located on the left hand side towards the back, you can actually see the location by the lime green area in the thermal image above.

The poe-less 5216-4ks2 uses an external power brick and the fan is about as quiet as you can get while still having one. Unscientifically, the top of the case and the air blowing out don't feel warm at all.
