What does that mean, and as oppose to what?

It means that Blue Iris will run in the background even if you close Blue Iris app.

as to running it only when you click the Blue Iris Icon.

As a service it will start running when the computer boots up and continues running. The data collects and alerts run as a service. The BI UI will only run when you click the icon.
