Files not being generated

This is something that has just happened. When BI triggers it goes through the motion of recording. It shows a picture of the recording in the catalogue on the right. When I try to view the recording it states that the bvr file is not found.

What I have done so far:
Rebooted Windows 2012 r2 server machine
Scanned the disk for errors
Ensured permissions where set properly

I'm at a lost where to begin looking, any help would be appreciated

Have you tried rebuilding the database? That's what I would try first.
